How the grade works
CMS overall rating (25), Mortality (25), Safety of care (25), Readmission (25). Each metric is percentile-ranked within the hospital's state and type (acute care, critical access, children's, psychiatric), weighted, and summed; A ≥ 72, B ≥ 58, C ≥ 42, D ≥ 28, F below. Hospitals with fewer than three of the four metrics are shown as "Not graded" — never an F by absence. Every hospital page prints the raw inputs, the percentiles and the peer group.
Mortality, safety and readmission are counted the way CMS reports them: how many of a hospital's measures are better than the national rate, and how many are worse. A hospital that is better on three measures and worse on none scores +3; one that is worse on two and better on none scores −2.
